Community Outreach
PARTICIPATION IN COMMUNITY SOCIAL PROGRAMS
Chesterfield plays its role in the enhancement of the quality of life through participation in various community based social programs.
By establishing close collaboration between its Case Managers and the Social workers, we have been able to assist the older adults and those with developmental challenges on making choices regarding home care options and applicable care plans.
Based upon such choices, the care recipients are able to access available medical assistance programs. In furthering its social community interventions, Chesterfield has been active in promoting outreach services pertaining to comprehensive care services in multicultural environments that comprise the communities it serves.
Many immigrant employees and their families have been assisted in obtaining immigration services and solutions to housing problems. By networking with religious organizations and other cultural groups a large number of immigrants have been successfully assisted in getting jobs.
Since July 2006, we have been involved jointly with the Department of Health in the administration of a research grant for a project which seeks to establish, at a snapshot, the status of the general health of Washington state residents.
Chesterfield is centrally placed in its efforts to facilitate the delivery of culturally responsive health care systems in the field of long term care which has not been given the prominent attention that it deserves.
